## Narrative

On November 3, 2017, an employee was working with another employee to unjam a wrapper machine. The first employee was pulling on the jammed film wrap while the other was operating the controls. The sealer bar lowered and amputated the first employee's left ring and index fingertips. The machine was guarded at the time.
