## Narrative

On November 14, 2017, at approximately 3:05 p.m., an employee was operating a stand-up reach lift in reverse when he turned sharply. The powered industrial truck then slammed into a guardrail. His left foot was crushed and fractured between the guardrail and the powered industrial truck, requiring hospitalization.
