## Narrative

An employee was harnessed to an order picker. He stepped off the order picker onto a forklift and started pulling away. The harness yanked him off the forklift back to the order picker causing a cracked vertebra in his neck, a  four-inch gash on his forehead and a small gash on his right thumb.
