## Narrative

An employee was testing the repair on the slide cylinder of a refuse truck by operating the packer panel lever with his left hand. When he engaged the lever, the cylinder linkage malfunctioned and caused the lever to abruptly kick in toward the side wall of the truck body crushing the upper half of the left index finger between the lever and side wall of the truck body resulting in a contusion and fracture.
