## Narrative

An employee was setting up a three-stand mill machine. As he placed his right hand on the block, his left hand pressed the jog forward button instead of the tension off button, causing the block to rotate counterclockwise and amputate his right middle and ring fingertips. The jog button was unguarded at the time.
