## Narrative

An employee was unjamming a box on a packing line from the mezzanine catwalk above the line. The employee stood up after unjamming the box and was struck by an overhead ceiling fan blade. The force of the strike caused the employee to land on the conveyor belt, which was locked/tagged out. The employee sustained a severe head injury, requiring staples.
