## Narrative

An employee was using an 8-foot fiberglass A-frame step ladder to access the ceiling. When he went to descend the ladder, it slipped. This startled the employee, who lost his balance and fell to the floor below. He fractured his left elbow and right wrist, requiring hospitalization.
