## Narrative

An employee was troubleshooting the shaker table on a plate machine. As the employee bent over to pick up a screw, the machine was activated catching her hand that had been placed on the table in the shaker table chain resulting in amputation of the right ring fingertip. The machine was unguarded at the time.
