## Narrative

An employee was operating a walkie pallet jack to pick up a load when she turned the pallet jack sharply. The pallet jack then increased speed and threw her into a support beam. She was pinned between the pallet jack and the beam, and as she attempted to get un-stuck, she accidentally hit the accelerator, causing the pallet jack to strike her again. She suffered a laceration to the right leg, pelvic fracture, and fracture to the L5 vertebra.
