## Narrative

On July 11, 2018, at approximately 5:00 PM, an employee was operating a riding pallet jack in the warehouse when the operating handle of the pallet jack went under a racking system. Her right middle finger was crushed between the handle and the rack, requiring hospitalization and surgery.
