## Narrative

On August 9, 2018, an employee was using an electric grinder to remove the welds from an elbow fitting that was strapped to a welding fixture when the elbow fitting kicked back and his fingers were pinched between the elbow and welding fixture, resulting in right index and middle fingertip amputations.
