## Narrative

On August 9, 2018,at approximately 12:25 a.m., an employee was walking along a grated walkway while inspecting fire extinguishers at a third-party facility when the grating gave way. He then fell approximately 20 feet and injured his back, requiring hospitalization and surgery.
