## Narrative

On August 15, 2018, an employee was operating a stand-up forklift and turning a corner when the vehicle hit a wet spot and started to slide. The employee released the dead man switch to stop the forklift and then dismounted the vehicle during the slide. His left foot/ankle was struck by the vehicle and a plastic pole protector as the vehicle continued to travel in the direction he exited. He suffered multiple fractures and a puncture wound.
