## Narrative

An employee was changing a conveyor chain inside a robot cell. He pushed the transmission case forward to take the chain off the sprocket. The transmission case activated a photo sensor, which pushed the next case up the line. The employee's right index finger was then pinched in the chain, resulting in a finger amputation. The conveyor was not locked out at the time of the incident.
