## Narrative

An employee was feeding material into the in-feed trough of a saw. While the material was being automatically fed by the in-feed device, his right hand came into contact with the activated blade, resulting in the amputation and partial amputation of his right index, middle, and ring fingers. The saw blade was guarded at the time.
