## Narrative

On September 26, 2018, an employee was pressure washing a truck conveyor when he tripped over the pressure washer hose and fell approximately 14 feet to the ground below. He was hospitalized with a dislocated left shoulder and fractures to the right arm, L1 vertebra, and pelvis.
