## Narrative

On October 17, 2019, at 9:50 a.m., an employee was clearing soft condiment from a blocked pipe on a displacement pump when his left index finger contacted the moving propellers inside the pipe, amputating his fingertip. The equipment was not de-energized and locked out at the time of the incident.
