## Narrative

An employee had her safety vest on and was using a walker stacker to pull down a pallet of bags from a steel rack. She backed up against a beam and the walker ran over her foot. Her foot became caught between steel and the walker, causing a compound fracture to the big and little toes. She was hospitalized and had surgery.
