## Narrative

An employee was working with two other employees to ensure the proper setup/startup of an extrusion machine.  The injured employee was conducting a quality check of the product on the conveyor between the extrusion and fibrillation roller when his left hand was caught between the conveyor roller and fibrillator, resulting in a left hand amputation.
