## Narrative

On November 26, 2018, an employee was helping a forklift driver move a 4,000-pound load of rubber when the forklift driver hit the tilt button, causing the load of rubber to shift. This caused the bar that was supporting the rubber to release from the forklift tines and strike the employee, who suffered skull, cheekbone, and neck vertebrae fractures; a broken femur; and a smashed third right toe. The employee required surgery and stitches to the back of the head.
