## Narrative

On November 28, 2018, an employee placed a check fixture on a rolling cart using an overhead hoist. As the employee was rolling the cart, the cart struck a seam in the concrete, causing the fixture to shift and crush the employee's left index finger, resulting in a fingertip amputation.
