## Narrative

At about 8:10 a.m. on December 11, 2018, an employee was adjusting material while on the bed of a flatbed truck. The employee slipped on frost and fell off the truck bed, landing on their feet on the ground (about 4 feet below) and breaking their right heel. The employee was hospitalized for surgery.
