## Narrative

On January 16, 2019, at approximately 4:00 PM, an employee was installing insulation to a steel exhaust vent in the storage garage as a temporary solution to reduce escaping heat. The employee was working from an aerial lift and was found unconscious and face down on the gravel surface below the lift. The lift was positioned in an elevated position with the basket door open. It is presumed that the employee fell from the lift. The employee was hospitalized for head trauma. Fall protection was not worn at the time.
