## Narrative

An employee was cleaning a moving table top (carrousel) on a concrete pipe manufacturing machine during set-up. As the employee went to clean the table skirt, his rag and a finger were pulled into the wheel. His right middle finger was crushed and the fingertip, up to the first knuckle, was surgically amputated.
