## Narrative

At 10:00 p.m. on April 1, 2019, an employee was on a pallet jack, transferring and lifting equipment. The jack lost power after hitting a bump in the floor, and the employee fell off it and struck his head on the concrete. He suffered a severe head injury and brain bleeding.
