## Narrative

An employee was exiting a trailer with a loaded pallet jack when a metal patch on the floor gave way a bit due to the weight of the loaded pallet jack. The employee lost control of the pallet jack and the handle struck him in the pelvis, causing a deep contusion that required hospitalization.
