## Narrative

An employee was loading a pallet of milk on a trailer. His pallet jack fork hit a piece of wood that had broken off from a pallet he had just loaded, causing the milk to start to tip over. The employee dismounted the pallet jack and tried to stop the pallet from falling. His finger was pinched between the falling pallet of milk and the side of the trailer, resulting in a left middle fingertip amputation.
