## Narrative

A boiler plug was leaking and the new plug would not seal due to galling on the threads. Employees tried to grind the threads straight. A boiler gate valve was closed, so the employees used a spray of penetrating oil. Sparks from the grinding caused flames to release from the boiler. The pressure forced an employee backwards and led to burns on both of the employee's arms.
