## Narrative

On June 14, 2019, an employee installed an adapter on a compressed gas cylinder, then actuated the cylinder. There was a suspected failure in the adapter, and the cylinder struck the employee. The employee suffered fractures to the ribs and shoulder, as well as a head injury.
