## Narrative

On July 1, 2019, an employee was using an air compressor to fill a forklift tire that was mounted to the forklift and attached to a single-piece rim wheel. While the employee was filling the tire it exploded and blew the employee back. The employee hit the wall and suffered a possible concussion.
