## Narrative

On 7/23/2019, an employee was attempting to remove a crate from a pile of stacked materials when a pile of steel beams became dislodged and fell toward the employee. The smaller steel beams struck a larger steel beam, which then fell onto the employee's right foot. The employee sustained multiple broken foot bones, requiring hospitalization and surgery.
