## Narrative

Employees were setting up for a stress test using a 6"x6" wooden piece of dunnage that was 8 feet long. They were using a forklift to assist in moving the dunnage and counterweights. When the forklift lowered its forks, the weight of the dunnage shifted and it went off balance. The injured employee's right middle finger was pinched between the ground and the end of the dunnage. The employee was hospitalized and his fingertip was medically amputated.
