## Narrative

An employee was cutting up a piece of equipment for removal. The employee cut a chain and then threw it over a de-energized sprocket. The counter weight yanked the chain and his fingers into the sprocket assembly, pinching the tips of his fingers. The pinky and ring fingertips on the right hand were amputated and further surgery was required on the middle finger.
