## Narrative

An employee was doing preventive maintenance on a gallon caser, cycling the cylinders to identify leaking cylinders or sticking solenoids. He noticed some buildup on the fingers of the machine and was trying to determine how much buildup was present when the fingers crushed his right index finger, removing about half his fingernail and some tissue above the bone.
