## Narrative

Three employees were trying to set up a concrete silo mixing plant and had the silo lifted on a forklift. The back legs on the silo needed to be adjusted to level out the silo. The legs were stuck, so the forklift was tilted forward to try and force the legs free by lowering it onto the ground. The silo slid forward and struck the employees, one of whom suffered a fractured leg and was hospitalized for surgery.
