## Narrative

An employee was standing on level ground, stocking cases of tissues. He became lightheaded, lost his balance and fell. He tried to catch himself on a forklift and his right forearm got stuck between a pallet and the forklift backrest. The employee suffered a fractured right forearm and was hospitalized.
