## Narrative

On May 07, 2020, at 8:30 a.m., an employee was operating a powered pallet jack when a co-worker operating another powered pallet jack struck his pallet jack, causing him to lose control of his pallet jack. He crashed into a rack, pinning his ankle between his pallet jack and the rack.
