## Narrative

An employee was standing on a 12-foot ladder, cutting a 3-inch water pipe. When he cut it, the pipe swung down and knocked the ladder out from under him. He fell to the ground about 9 feet below, landing on the ladder and suffering rib, lung, and hip damage.
