## Narrative

Employees were erecting scaffolding inside a vessel. An employee climbed up from near the bottom to the top of the vessel (at ground level). As he was walking up a sloped duct, he lost his balance and fell, sliding about 15 feet down. He suffered a head contusion and an injury to the side.
