## Narrative

On September 24, 2020, at approximately 11:00 a.m. an employee was walking from an area to the next well. A guide bar for the rear drilling area was lodged on a matting board. As the employee attempted to dislodge the guide bar, he was struck by the bar resulting in a fractured lower right leg.
