## Narrative

An employee was using a press brake to form 2-by-2 metal sheets into a box. As he was retrieving a metal piece, his right hand was caught in the point of operation as the press brake activated. It closed on his finger tips, causing partial fingertip amputations to the right index, middle, and ring fingers.
