## Narrative

An employee was working on the empty tote conveyor. As he was putting a chain onto a sprocket, a box passed through a photo eye, triggering the conveyor.  The employee was caught by the sprocket and chain, sustaining the amputation of 1/8" of the left ring fingertip.
