## Narrative

On November 11, 2020, three employees were stacking two crates on top of each other for transport to a mulching station. One employee was on one end and the other two were on the other; when the top crate was pushed toward the single employee, the employee's left index finger was caught, fractured, and partially amputated between the two crates.
