## Narrative

At 10:00 a.m. on January 6, 2021, an employee was operating a 55-gallon drum crusher when the ram of the crusher pushed the  -inch metal door outward, breaking the door welds and striking the employee in the head. The employee suffered head trauma and was hospitalized.
