## Narrative

An employee was operating a hydraulic brake press to shape a piece of sheet metal. The employee was manually positioning the workpiece as he operated the machine and his left hand became caught between the die and the top punch of the brake press, resulting in amputation of the distal phalanx of the left index finger.
