## Narrative

An employee was assisting a co-worker in opening a precast concrete form so that the outer shell could be removed from the form when his left hand became caught between the form and the shell. The employee suffered a crushed fingertip, causing a fracture of the distal phalange, and partial amputation of the middle fingertip.
