## Narrative

An employee working in a home under construction was prepping trim on a ladder that was on top of scaffold. The scaffold tipped over while he was on the ladder and the employee fell to the floor, resulting in a broken right elbow, broken left wrist, cut nose, and contusion above the right eye.
