## Narrative

At about 6:45 a.m. on Tuesday, May 25, 2021, two employees were removing a shaft from a plastic sheet roll core. One of them was releasing air from the shaft air bladders when her right index finger was caught between the shaft and the core. The fingertip was amputated.
