## Narrative

At about 11:30 a.m. on July 19, 2021, an employee was on a ladder when the ladder slipped out at the bottom. The employee fell to the concrete 5 or 6 feet below, landing on their right side and suffering fractures to the right foot and shoulder. The employee was hospitalized.
