## Narrative

An employee removed a flag pin to replace a hydraulic cylinder during servicing of a loader. The upper part of the machine frame separated, causing a hinged coupler to fall to the floor from an "upward" position, crush the employee's feet, and knock him down. He suffered fractures to both feet and a broken wrist.
